Module 2 Assignment

Objective: 

• Complete an initial assessment for a resident who is non-verbal. 
• Complete the “F Section” Observation Worksheet. 
• Write a concise activities narrative that describes unique attributes of a resident essential for caregivers.
• Write a comprehensive care area assessment for a resident who is nonverbal in a manner that applies effective documentation standards. 

Assignment Steps:

1. Select a fictional character from a movie or story of your choice. 
2. Imagine that this person has moved into your nursing home and you need to complete an initial assessment for them. They are non-verbal, so you must research their past using an internet search to complete your evaluation. 
3. Download and print the “Section F Observation” Worksheet and complete it for this resident.
4. Download and print the “Person-Centered Initial Assessment” form and complete it for this resident. 
5. Write an Initial Activities Narrative and Care Area Assessment for this resident.
6. Your activities narrative must not exceed 150 words, and include the following:
   1. A brief overview of the resident.
   2. At least two important things you feel a CNA should know about the resident that are not listed in the initial assessment.
   3. At least one care planning consideration.
7. Your Care Area Assessment must include the following:
   1. The triggered area that requires further assessment.
   2. Causes and contributing factors related to the triggered area.
   3. Risk factors related to the triggered area.
   4. A decision to care plan or not care plan for the triggered area.
   5. Reasoning the care plan will or will not be developed for this triggered area.
