Care Planning with Confidence

Objective:

  • Write a care plan for a resident who is nonverbal.
  • Write a care plan focus statement that describes a problem, the reason for the problem, and how the problem impacts the resident’s engagement.
  • Write a care plan goal statement describing the resident’s action, condition, reason, and time frame.
  • Write a care plan approach/intervention statement that is individualized, realistic, and specific.

Assignment Steps:

  1. Using what you wrote from your previous assignment, create a care plan for your fictional character. Your care plan should include, for each triggered area that you decided to care plan for in the care area assessment:
    1. At least one focus area.
    2. At least one goal statement.
    3. At least one intervention.
  2. Submit your completed care plan as a Word/Google Document, Graphic Organizer, or PowerPoint/Google Slides presentation by uploading it to this assignment block.

Focus Statements The Focus statement is related to a triggered area that was determined to be care planned for OR is something the resident has identified as very important.The Focus statement describes the problem, the reason for the problem, and what the problem looks like concerning the resident’s needs.      
Goal Statements The goal statement describes what the resident will do, by what condition, for what reason, and by when.The goal statement is specific, measurable, achievable, realistic, and time-oriented.      
Approach Statements Approaches are individualized for the resident.Approaches are realistic, specific, and clear.
